Erectile dysfunction: Key Takeaways

  • Erectile dysfunction (ED) is the persistent inability to get or keep an erection firm enough for sex. It is common, not a personal failing, and it is treatable.
  • A large US study estimated that ED affects about 18 million American men aged 20 and over (18.4%), and a widely cited projection puts the global burden at 322 million men.
  • ED can be an early warning of heart disease and diabetes. In a trial of 9,457 men, those with ED faced a 45% higher risk of later cardiovascular events.
  • The five signs below deserve a doctor’s visit if they last more than a few weeks.
  • Most men can regain function through lifestyle change, medication, therapy or a mix of all three.

What Is Erectile Dysfunction?

Erectile dysfunction is the regular difficulty in achieving or maintaining an erection sufficient for satisfactory sexual activity. Clinicians prefer the term to the older word “impotence” because it is more precise and carries less stigma. An occasional bad night is normal. Erectile dysfunction is diagnosed when the problem is recurring, usually over three months or more.

The condition is far more widespread than the silence around it suggests. A 2007 analysis published in the American Journal of Medicine found a prevalence of 18.4% among US men aged 20 and over. Researchers at the Johns Hopkins Bloomberg School of Public Health, who led the work, noted that only about 5% of men aged 20 to 40 were affected, versus a much larger share of men 70 and older. Estimates vary widely worldwide, from roughly 3% to 77% depending on the population and diagnostic method, according to a 2019 review in BJUI.

5 Signs of Erectile Dysfunction You Should Not Ignore

1. Persistent trouble getting an erection: The most recognisable sign is repeated difficulty starting an erection when you are aroused. If it happens in most attempts for several weeks, rather than once during a stressful week, it is time to speak to a healthcare provider. Because an erection depends on healthy blood flow, nerves and hormones, a consistent failure can point to a physical cause.

2. Difficulty keeping an erection: Some men achieve an erection but lose it before or during intercourse. This pattern, sometimes called erection “leakage”, is a classic sign of reduced blood flow to the penis, a problem often linked to narrowed or stiffened arteries.

3. Erections that are softer or less firm than before: ED is not always an all-or-nothing problem. Many men first notice that erections take longer, need more direct stimulation, or are not as rigid as they used to be. Doctors grade ED as mild, moderate or severe, and early treatment is most effective at the mild stage. In one study of 2,660 sexually active men aged 18 to 31, about 11% reported mild ED and nearly 3% moderate to severe ED, a reminder that this is not only a problem of older age.

4. Fewer or absent morning erections: Men normally have several erections during sleep, which is why many wake with one. A marked drop in morning or night-time erections can suggest a physical contributor such as vascular disease, low testosterone or nerve damage. If erections still occur on waking but fail with a partner, stress or anxiety may be playing a larger role. This is a useful clue, not a diagnosis, so share it with your doctor.

5. A drop in sexual desire, or avoiding intimacy: Low libido can accompany ED, particularly when testosterone is low or when depression and anxiety are involved. Many men also begin to dodge sex because of fear of failure, which in turn worsens the problem. Research in the Journal of Sexual Medicine (2018) found that depression and ED influence each other in both directions, so each can make the other worse.

Why ED Can Be a Warning Sign for Your Heart

The arteries supplying the penis are much narrower than those feeding the heart, so they can show damage first. In the placebo group of the Prostate Cancer Prevention Trial, which followed 9,457 men aged 55 and older, men with ED had a hazard ratio of 1.45 for subsequent cardiovascular events. Researchers said the association was in the range seen with current smoking or a family history of heart attack, and concluded that ED should prompt a check of cardiovascular risk factors.

A separate US analysis found that almost 90% of men with ED had at least one cardiovascular risk factor, such as diabetes, high blood pressure, poor cholesterol levels or smoking.

Main Causes of Erectile Dysfunction

ED usually has more than one cause. Common contributors include:

• Cardiovascular disease and high blood pressure: prevalence of ED in men with hypertension is reported at roughly 60%.

• Type 2 diabetes: the 2007 US analysis found a 51.3% prevalence of ED among men with diabetes; a 2024 global review put it near two in three.

• Chronic kidney disease: reported in around 80% of men with the condition.

• Obesity, smoking, heavy alcohol use and inactivity: the Hopkins researchers linked a lack of physical activity independently to ED.

• Low testosterone and other hormonal problems.

• Medications: some blood pressure drugs, antidepressants and others can contribute. Never stop a prescription without medical advice.

• Psychological factors: stress, depression, anxiety and relationship strain.

• Surgery or injury: particularly pelvic or prostate procedures and spinal injury.

How Is ED Diagnosed?

A doctor will typically take a medical and sexual history, check blood pressure, and order blood tests for glucose, cholesterol and testosterone. Questionnaires such as the International Index of Erectile Function (IIEF-5) grade severity. Because ED can reveal hidden disease, a good evaluation looks at the whole body, not just the bedroom.

ED Treatment and Management Options

1. Lifestyle changes. These help most men and benefit overall health: regular aerobic exercise, weight loss if overweight, stopping smoking, limiting alcohol, and managing blood sugar, cholesterol and blood pressure.

2. Oral medicines (PDE5 inhibitors). Sildenafil, tadalafil, vardenafil and avanafil are first-line treatments for most men. They need sexual stimulation to work and are not safe with nitrate medicines used for chest pain, so tell your doctor about everything you take.

3. Other medical treatments. Options include alprostadil (injection or urethral), vacuum erection devices, testosterone therapy where deficiency is confirmed, and, in selected cases, penile implants.

4. Counselling and sex therapy. Cognitive behavioural therapy and couples counselling are effective when stress, anxiety or relationship issues contribute, and they often work best alongside medical care.

5. Treat the underlying condition. Controlling diabetes, hypertension or heart disease can improve erectile function.

Beware of unregulated “herbal” pills and online products sold without a prescription. Some have been found to contain undeclared drugs.

When to See a Doctor

Book an appointment if symptoms last more than a few weeks, if they started suddenly, or if you have diabetes, heart disease or high blood pressure. Seek emergency care for an erection lasting more than four hours, or if ED comes with chest pain, shortness of breath or dizziness.

Only an estimated one in four men with ED receives treatment, according to figures cited by SingleCare, so delay is the norm. It should not be.

Frequently Asked Questions

What is the most common early sign of erectile dysfunction? Trouble keeping an erection firm enough for sex, or needing more stimulation than before, is typically the earliest sign.

Can young men get erectile dysfunction? Yes. Studies of men in their late teens to early 30s show a smaller but real prevalence, often tied to stress, anxiety, smoking, substance use or obesity.

Is erectile dysfunction a sign of heart disease? It can be. ED is recognised as an independent marker of cardiovascular risk, which is why doctors check heart health when ED appears.

Can erectile dysfunction be reversed? Often, yes. Many men improve with lifestyle change, treatment of underlying conditions, medication or therapy.

Does stress cause erectile dysfunction? Stress and anxiety can cause or worsen ED, but persistent problems should still be checked for physical causes.

Bottom Line

Erectile dysfunction is common, treatable and sometimes a canary in the coal mine for serious illness. Men who notice any of these five signs should act early rather than suffer in silence.
